Vision Principles: <br />Neighborhoods and a Sense of Community, Quality of Life and Lifelong Learning <br />Community. <br />Background: <br />• Since 1993, the Department of Behavioral Healthcare Services (BHS) provides <br />permanent supportive housing to Portsmouth's homeless citizens with mental illness, <br />chronic substance abuse addictions, intellectual and developmental disabilities, and/or <br />HIV/AIDS through the United States Housing and Urban Development (HUD) Shelter <br />Plus Care (S+C) grant. <br />• As part of its program, BHS must provide case management. Using grant funds, <br />BHS employs two housing case managers to administer the S+C grant. Since <br />inception, S+C grew from serving 30 to over 70 individuals per year in single and family <br />households. Case Managers assist homeless individuals in accessing Shelter Plus <br />Care housing units, meet monthly with the residents in the Shelter Plus Care supported <br />housing units, connect these citizens and families to support services, negotiate and <br />resolve issues between landlords and tenants. <br />• In addition, Case Managers provide required HMIS documentation, perform HUD <br />required visual inspections of the Shelter Plus Care units, and maintain the HUD <br />required documentation of support services. <br />• In January 2014, BHS submitted renewal grants to HUD for S+C in two parts, as <br />in previous years. The first part is the direct housing services portion of S+C in the <br />amount of $457,188 with the $114,297 required local match. The second part is the <br />case management portion known as Supportive Housing Program (SHP) for $69,013 <br />with a $17,297 required local match. <br />
