Vision Principle: <br />Neighborhood and Community Transformation. <br /> <br />Background: <br />? <br /> Discharge Assistance Project (DAP) funds are utilized to purchase services and <br />equipment to enable disabled consumers who require such services in order to be <br />discharged to the community. Community Services Board (CSB) Case Managers <br />submit DAP plans on specified consumers hospitalized at Eastern State Hospital (ESH) <br />to the Regional Discharge Assistance Committee (REDAC) for review and approval of <br />funds for services requested. <br /> <br />Discussion: <br />? <br /> Two Mental Retardation Services consumers have been approved by REDAC for <br />$37,185 to purchase needed services to discharge them to the community from ESH. <br />? <br /> One Mental Health Services consumer has been approved by REDAC for <br />$21,600 to purchase needed services to discharge her to the community from ESH. <br />? <br />Financial Impact: <br /> There is no financial impact to the City. <br /> <br />Staff Recommends: <br />Adoption of an Ordinance to appropriate $58,785 of Discharge Assistance Project <br />funding to the Department of Behavioral Healthcare Services to assist consumers upon <br />discharge from the Eastern State Hospital to reside in the community.
