<br />a food service employee or food service manager card issued by the director of public health of the <br />city or such director's authorized representatives. <br /> <br />(b) It shall be unlawful for any person to employ or permit another person to remain <br />employed in any place or establishment referred to in subsection (a), above, unless such person <br />holds a valid food service card issued pursuant to this chapter. <br /> <br />(c) Food service cards required by this chapter shall be issued by the director of public <br />health only to food service employees and food service managers who shall furnish such <br />information and laboratory specimens and submit to such physical examination and tests as the <br />director may require and/or who have received such instructions in personal hygiene and food <br />establishment sanitation as may be offered or required by the director; provided, however, that the <br />director may, when in the director's judgment it seems proper, issue a temporary card to an <br />applicant. Such temporary card shall be effective only for so many days as the director shall indicate <br />thereon in writing, not to exceed 30 days. <br /> <br />(d) A food service card issued by another city, county or organization within the <br />Commonwealth shall be honored in the city until its expiration, provided that requirements to obtain <br />such card were equal or greater than those set forth in this article. Determinations concerning such <br />requirements shall be made by the director. <br /> <br />(e) Such fees as the director deems appropriate for each food service card shall be paid <br />by the applicant to the health department cooperative budget to defray the necessary administrative <br />expenses. <br /> <br />(f) Each food service card issued shall remain in force until date of expiration or until <br />suspended or revoked by the director for cause. Temporary food service cards, referred to in <br />subsection ( c), above, and food service cards accepted by reciprocity as set forth in subsection (d), <br />above, may also be suspended or revoked by the director for cause. <br /> <br />(g) A food service card issued under the provisions of this chapter shall be produced by <br />the food service employee or food service manager for inspection upon the request of the director. <br /> <br />(h) Upon the expiration of the food service card, the holder thereof may renew the same <br />by payment of the fee prescribed by subsection (e), above, and by successfully completing such <br />training classes and/or tests as the director directs. The date, time and location of said classes or <br />tests shall be determined by the director of public health. <br /> <br />(i) A food service card issued under this chapter shall not be transferred to or used by <br />any person other than the person to whom it is issued. The food service card is the property of the <br />employee. <br /> <br />G) The director shall have the authority to waive the requirements of this section for <br />special events of a temporary nature. Such waiver shall be in writing. <br />
