a food service employee or food service manager card issued by the director of public health of the city or such director's authorized representatives. (b) It shall be unlawful for any person to employ or permit another person to remain employed in any place or establishment referred to in subsection (a), above, unless such person holds a valid food service card issued pursuant to this chapter. (c) Food service cards required by this chapter shall be issued by the director of public health only to food service employees and food service managers who shall furnish such information and laboratory specimens and submit to such physical examination and tests as the director may require and/or who have received such instructions in personal hygiene and food establishment sanitation as may be offered or required by the director; provided, however, that the director may, when in the director's judgment it seems proper, issue a temporary card to an applicant. Such temporary card shall be effective only for so many days as the director shall indicate thereon in writing, not to exceed 30 days. (d) A food service card issued by another city, county or organization within the Commonwealth shall be honored in the city until its expiration, provided that requirements to obtain such card were equal or greater than those set forth in this article. Determinations concerning such requirements shall be made by the director. (e) Such fees as the director deems appropriate for each food service card shall be paid by the applicant to the health department cooperative budget to defray the necessary administrative expenses. (f) Each food service card issued shall remain in force until date of expiration or until suspended or revoked by the director for cause. Temporary food service cards, referred to in subsection ( c), above, and food service cards accepted by reciprocity as set forth in subsection (d), above, may also be suspended or revoked by the director for cause. (g) A food service card issued under the provisions of this chapter shall be produced by the food service employee or food service manager for inspection upon the request of the director. (h) Upon the expiration of the food service card, the holder thereof may renew the same by payment of the fee prescribed by subsection (e), above, and by successfully completing such training classes and/or tests as the director directs. The date, time and location of said classes or tests shall be determined by the director of public health. (i) A food service card issued under this chapter shall not be transferred to or used by any person other than the person to whom it is issued. The food service card is the property of the employee. G) The director shall have the authority to waive the requirements of this section for special events of a temporary nature. Such waiver shall be in writing.
