<br />. <br /> <br />. <br /> <br />. <br /> <br />legally made available by the tattoo artist unless such person holds the appropriate license <br />from a Virginia health regulatory board. <br /> <br />C. Records for each patron, and all other written records required by this chapter, shall <br />be maintained by the body art studio operator for a minimum period of four years. At such time <br />as the body art studio ceases doing business or is removed from the city or has a change in <br />ownership, copies of all such records shall be provided to the Health Department. The records <br />shall be subject to inspection by the Health Department at any time and shall include the <br />following information: <br /> <br />(I) Name, address, telephone number, sex, age, date of birth, and type and copy of <br />identification provided by the person receiving body art; <br /> <br />(2) Date of body art; <br /> <br />(3) Physical location and description of body art; <br /> <br />(4) Name, address and telephone number of the person performing the body art; <br /> <br />(5) For tattooing, name and address of the manufacturer of the dyes used as well as <br />identifying information about the dye solutions and types of dyes used. For piercing, the <br />content and type of body jewelry used. If a customer has need for this information, then <br />the body art operator shall release it to the customer without charge. <br /> <br />(6) Sterilization log number (in client record) of all re-usable instruments used on the <br />client. <br /> <br />(7) Consent form with signatures of the artist, client, and if applicable, parent or guardian, <br />and a statement that the client has received a copy of applicable written care instructions, <br />and that the client has read and understands the instructions. <br /> <br />(8) Any report of an adverse event or adverse reaction during or following body art <br />procedure. <br /> <br />D. No body art shall be done in any location, which, even though inside a body art <br />studio, is visible from a public way, street, or sidewalk. <br /> <br />E. No person shall perform body art, and no body art studio, shall be open for business <br />earlier than 7:00AM or later than 2:00AM. Provided, however, that if body art is commenced <br />prior to 2:00AM, then it may be completed, even if after 2:00AM. In any such case, the burden <br />
