Section 3-33° Application for permit; contents. Application for such permit shall be made to the City Manager upon a form supplied for that purpose. The appli- cation~shalI.contain the following information: (a) Name and address of the applicant, age, date and place of birth, and where pertinent, the relationship of the applicant to the company or corporation owning or managing the center. If a corporation, this information for the cor- porate officer signing the application, plus name and address of the registered ~gDn~, ~ate~of incorporation and state of incorporation, and names of officers and directors thereof. Name and address of the owner of the machines and owner of the premises if different from that of the above. (b) The location of the center and ~he business, if any other, conducted at the location. (c) Maximum number of patrons center will accommodate. (d) Maximum number of machines Center will accommodate and submission of necessary information in order to determine if there is sufficient space for such machines to prevent over crowding of users or interference with pedestrian circulation on the rest of the premises. (e) A certification by the applicant that all requirements of this Article have been complied with. (f) A list of all employees who are expected to serve in a managerial capaci'ty in the center. Prior convictions of the applicant and managerial employees as well as any~ pending criminal cases involving such persons. (g) A brief set of house rules which the managerial personnel shall enforce, both within the center and in the immediate area outside the center, such rules being designed to promote good order and peaceful behavior by the patrons of the center. (h) The responsibility to provide the information re- quested herein is regarded as continuing beyond the date of the application and the applicant is required to provide, by filing an amended application, any additional information which will augment or otherwise modify the information provided with the application. Such information shall be provided to the City Manager within ten (10) days after any chang~ from the information previously furnished.
