division which has been lost, stolen or destroyed may be <br />replaced by the City Manager upon application by the permittee <br />for a replacement permit in the same manner as is required <br />under Section 37-92, and upon the payment-of ha replacement fee <br />of five dollars ($5.00). In addition to the information <br />required by such section, such application shall include a <br />sworn statement setting forth the facts surrounding such loss, <br />theft or destruction. Any such replacement permit shall be <br />issued only for the remainder of the term of the lost permit. <br />Upon the issuance of a replacemen% permit, the lost, stolen or <br />destroyed permit shall be deemed cancelled, and a notation of <br />such cancellation shall be entered on the appropriate records. <br /> <br />Section 37-101. Revocation ~ Generally. <br /> <br /> (a) The City Manager shall revoke the public vehicle <br />driver's permit issued under this division upon receiving a <br />record of the final conviction of such permittee of any of the <br />following: <br /> <br />(1) Any felony. <br /> <br />(2) Driving a motor vehicle under the influence <br /> intoxicants or any controlled substance. <br /> <br />(3) Petit larceny. <br /> <br />of <br /> <br />(4) <br /> <br />Two (2) offenses of reckless driving or three (3) <br />offenses of speeding within the preceding twelve <br />(12) months. <br /> <br />(5) <br /> <br />Violation of any state law or municipal or county <br />ordinance relating to prostitution, sexual <br />intercourse, pandering, soliciting for immoral <br />purposes or permitting occupants of such public <br />vehicle to engage in such practices or transporting <br />passengers to houses of ill fame, assignation or <br />prostitution. <br /> <br />(6) Willfully charging and collectin9 fares in excess of <br /> the lawful rates prescribed by this article. <br /> <br />(7) <br /> <br />Violation of any state law or municipal ordinance <br />relating to the sale or handling of intoxicating <br />beverages. <br /> <br />(8) <br /> <br />Three (3) offenses of violating any provision of <br />this article or any other state, federal or city <br />ordinance as now in effect or as may be hereafter <br />amended and put into effect all within the preceding <br />twelve (12) months. <br /> <br /> <br />
