(13) Failure to have each public vehicle in operation a <br /> minimum of thirty (30) days per calendar quarter. <br /> <br />Upon failure of the owner or operator to rectify any condition <br />described above or to comply with any administrative <br />requirements of this division within one hundred eighty (180) <br />days from the date of the suspension, the certificate of <br />public convenience and necessity shall be revoked. <br /> <br />Section 37-91. Required. <br /> <br /> (a) No person shall drive a public vehicle in the city <br />unless that person has a current permit issued pursuant to <br />this division. Such permit sh~I1 be known as a "public <br />vehicle driver's permit" and shall be in addition to any other <br />license required by this Code or state law. <br /> <br /> (b) No public vehicle owner shall permit such vehicle to <br />be driven by any person who does not possess the permit <br />required by this section. <br /> <br />Section 37-92. Application. <br /> <br /> The applicatioa for a permit required by this division <br />shall be in writing, sworn to and presented to the City <br />Manager, and shall state %he full name, residence, places of <br />residence previous to moving to the applicant's present <br />address, age, height, color of eyes and hair, sex, place of <br />birth, places of previous employment of applicant, whether the <br />applicant is married or single, length of residence in the <br />city, whether or not the applicant is a citizen of the United <br />States, whether or not the applicant has been previously <br />employed or licensed as a driver or chauffeur, and if so, <br />whether the applicant's driver's license or permit has ever <br />been suspended, revoked, or refused in this or any other <br />state, whether or not the applicant is addicted to the use of <br />intoxicating liquors or drugs, and such other information as <br />may be required by the City Manager. The application shall be <br />endorsed by two (2) citizens of the United States who <br />personally know the applicant but are not family members or <br />prospective employers of the applicant. Any false statement <br />made in any such affidavit shall be promptly reported by the <br />City Manager to the Commonwealth's Attorney. <br /> <br />Section 37-93. Investigation of applicant. <br /> <br /> The City Manager or his duly authorized agent shall, upon <br />the filing of an application for a public vehicle driver's <br />permit under this division, cause to be made such examination <br />as he may deem proper relating to the qualifications and <br />fitness of the applicant to operate a public vehicle. <br /> <br /> <br />
