UP-95-15 <br /> <br /> EXHIBIT A <br /> <br /> NARRATIVE <br /> <br /> RAY'S RESIDENTIAL CARE HOME <br /> <br /> Portsmouth, Virginia 23704 <br /> (804) 397-6084 <br /> <br /> Ray's Residential Care Home, located at 1704 Bain Street, is <br />an assisted care home for ambulatory seniors 55 years or older, <br />and for special categories of persons with some limits in ability <br />of self care, but where medical care is not a major element. Our <br />Goal is "Quality Care". <br /> <br /> The home will house 10 persons and will be a family <br />oriented/permanent home for those who can no longer live or want <br />to live alone, and need limited assistance with activities of <br />daily living. Quality care is our goal. The residents will <br />receive residential care, and 24 hour supervision. The program <br />is designed to help the residents maintain, as nearly as possible <br />a normal life style. The program will be flexible enough to <br />accommodate different interests and needs. Nutritious meals, <br />planned activities, and individualized personal care are all part <br />of the home's program. <br /> <br /> Ray's Residential Care Home is a two-story dwelling with 10 <br />rooms plus two full bathrooms. Five of the rooms will be used as <br />bedrooms, three spacious multi-purpose rooms for passive <br />recreation and two kitchens, two screen-in porches, and one open <br />porch and green area for passive recreation. This structure <br />meets the State standards that require space for sitting, <br />visiting, dining and recreatIon. <br /> <br /> Ray's Residential Care Home is pleased to offer its <br />resIdents a home that can enhance their lives an a convenient <br />location in the well established community in West Park View. <br /> <br /> Nutritious meal will be prepared by staff. Food preferences <br />of the residents will be considered when menus are planned. <br />Resident's religious dietary practices will be respected. Snacks <br />will be available twice daily. All food consumption will be <br />limited to the dining area. The menu will be posted on a weekly <br />basis. <br /> <br /> Ray's Residential Care Home is assessable via Interstate <br /> 264, Midtown and Downtown Tunnels, five minutes from Portsmouth <br /> General and Maryview Hospitals. <br /> <br /> <br /> <br /> <br />
