UP-95-15 <br /> <br /> EXHIBIT A <br /> <br /> Prior to admission, a physlclan examination must be <br />submitted. Each report must be signed by the licensed physician, <br />the physicians designee, or an official o£ a local department of <br />health. Medical emergencies will be handled as per State <br />regulations. <br /> <br /> Each resident must have a History and Physical examination <br />completed with 30 days prior to admission. This examination <br />should also include a thorough check for communicable disease and <br />any other medical problems. Once a resident is admitted to Ray's <br />Residential Care Home, a physical examinat!on will be done once. <br /> <br /> This will be a Smoke free home. There will be no <br />smoking allowed on the premise. <br /> <br /> Visitors are encouraged and welcomed, visiting hours will be <br />from 9 a.m. to 7:00 p.m. with respect for meal times. <br /> <br /> Families of residents are requested to coordinate with the <br />Administrator on all leave time. When a resident leaves the home <br />on a leave of absence (LOA), the resident or responsible party. <br />must sign out and have a staff member initiate the sign out log. <br />If for any reason the resident is unable to return at the "time <br />in" stated, the resident or responsible party must telephone and <br />advise staff of change in schedule. <br /> <br /> Activities will be planned and implemented by the staff. <br />Activities will be posted on a weekly basis for review by the <br />residents. Each resident is encouraged, but never forced to <br />participate. Community meetings are held weekly to give all <br />residents the opportunity to discuss any problems they may have <br />or to just share a little of their activities. This time will <br />also be used to allow the resident to suggest any activities or <br />events that they m!ght want to attend, special menu planning, <br />holiday and birthday celebrations. On occasions, the staff will <br />invite guests from throughout the community who may have <br />important information to pass on to our residents. <br /> <br /> Monthly fire drills will be held for staff and residents. <br />Everyone is required to participase. <br /> <br /> <br /> <br /> <br />
