operate a public vehicle, he or she shall refuse to issue such permit. <br /> <br />Sec. 37-96. Form. <br /> <br /> All permits issued under the provisions of this division shall be in such form as the chief <br />of police may prescribe and shall contain the photograph and signature of the permittee and such <br />other information as the chief of police may prescribe. <br /> <br />Sec. 37-97. Term. <br /> <br /> A public vehicle driver's permit issued under the provisions of this division shall be <br />effective for one (1) year from the date of issuance unless sooner suspended, revoked or <br />canceled. <br /> <br />Sec. 37-98. Display. <br /> <br /> Each driver holding a public vehicle driver's permit issued under this division shall keep <br />posted such permit and photograph in the public vehicle operated by him so as to be plainly <br />visible to the occupants of such vehicle. <br /> <br />Sec. 37-99. Renewal. <br /> <br /> A public vehicle driver's permit issued under this provision may be renewed by the chief <br />of police by appropriate endorsement thereon upon the payment of a fee of ten dollars ($10.00); <br />provided, however, that the applicant for such renewal shall, at the time thereof, be qualified <br />under the provisions of this division to have issued to him an original public vehicle driver's <br />permit. <br /> <br />Sec. 37-100. Replacement when lost, stolen or destroyed. <br /> <br /> Any public vehicle driver's permit issued under this division which has been lost, stolen <br />or destroyed may be replaced by the chief of police upon application by the permittee for a <br />replacement permit in the same manner as is required under section 37-92. and upon the payment <br />of a replacement fee of five dollars ($5.00). In addition to the information required by Section <br />37-92, the application shall include a sworn statement setting forth the facts surrounding the loss, <br />theft or destruction. Any such replacement permit shall be issued only for the remainder of the <br />term of the lost permit. Upon the issuance of a replacement permit, the lost, stolen or destroyed <br />permit shall be deemed cancelled, and a notation of such cancellation shall be entered on the <br />appropriate records. <br /> <br />Sec. 37-101. Ineligible persons. <br /> <br /> The chief of police shall not issue a public vehicle driver's permit to an applicant who <br />meets any of the following denial criteria: <br /> <br /> 13 <br /> <br /> <br /> <br />
