093 <br /> April 11, 1995 <br /> <br /> WHEREAS, it has been determined to be in the best interest <br />of the city of Portsmouth and its elderly population that the <br />Department of Social Services serve as the lead agency in <br />coordinating suCh loCal long-term care services and that the <br />Portsmouth Department of Public Health, the Portsmouth Community <br />Services Board, the Southeastern Virginia Area-Wide Model <br />Program for Seniors (SEVAMP), the Mayor's Senior Citizens <br />Committee, the Portsmouth Redevelopment and Housing Authority <br />(PRHA), and the local Nursing Home Pre-Admissions Screening Team <br />serve as member agencies in such coordination efforts; and <br /> <br /> WHEREAS, it is required by Section 2.1-373.7 that the lead <br />and member agencies establish a long-term care coordination <br />committee composed of, but not limited to, representatives of <br />the lead and member agencies to guide the coordination and <br />administration of public long-term care services for the elderly <br />in the City. <br /> <br /> NOW, THEREFORE, BE IT RESOLVED that the Council of the City <br />of Portsmouth, Virginia, hereby designates the Portsmouth <br />Department of Social Services as the lead agency and the <br />Portsmouth Department of Public Health, the Portsmouth Community <br />Services Board, the Southeastern Virginia Area-Wide Model <br />Program (SEVAMP), the Mayor's Senior Citizens Committee, the <br />PRHA, and the local Nursing Home Pre-Admissions Screening Team <br />to coordinate local long-term care services for the elderly, <br />having the particular responsibility under Section 2.1-373.7 of <br />the Code of Virginia of establishing a long-term care <br />coordination committee with duties as set out above and in said <br />statute." <br /> <br />Ayes: Clsmons, Griffin, Hawks, Martin, Pitts, Rohinett, Webb <br />Nays: None <br /> <br />95-185 - Letter from the City Manager recommending adoption of a <br />motion to transfer $30,000 from the Grants Contingency Account <br />to assist eligible Portsmouth residents in the purchase of their <br />prescription medications through the Medication Access Program <br />for Portsmouth (MAPP). <br /> <br /> "The Portsmouth Department o~Social Services has received <br />a grant in the amount of $30,000, divided into four quarterly <br />payments of $7,500 each. The grant is~to assist eligible <br />Portsmouth residents in the purchase of their prescription <br />medications through the Medication Access Program for Portsmouth <br />(MAPP). Residents/clients will be referred to either of five <br />different pharmacies, depending on which one is closer to the <br />person's home. There is no local match requirement for this <br />grant. <br /> <br /> I hereby recommend the approval of the program and the <br />transfer of $30,000 from Grants Contingency Account to support <br />this program." <br /> <br /> Motion by Mr. Clemons, and seconded by Mr. Pitts, to <br />transfer $30,000 from~the Grants Contingency Account to assist <br />eligible Portsmouth residents in the purchase of their <br />prescription medications, and was adopted by the following vote: <br /> <br />Ayes: Clemons, Griffin, Hawks, Martin, Pitts, Robinett, Webb <br />Nays: None <br /> <br />- NEW BUSINESS <br /> <br />95-186 - Mr. Clemons offered the following nominations for <br />appointments to the following Boards and Commissions: <br /> <br />Museum and Fine Arts Co~sslo~ <br /> <br />Herman Bates to be reappointed for a three year term ending <br />November 1, 1997. <br /> <br /> <br />
