December 11, 2001 <br /> <br />The grant requires a 25% local cash match for each year. The department of <br />Behavioral Healthcare will use these funds from this three-year grant for the <br />expansion of a special case management unit to provide for the coordination, <br />linkage and monitoring of supportive services to serve the City's disabled <br />citizens. The local cash match of $18,116 for the first year has been budgeted. <br /> <br />Recommendation: <br /> <br />· Adoption of a motion to transfer $271,740 from the Grants Contingency Account for <br />a three-year Supportive Housing Program (SliP) grant awarded by the Department of <br />Housing and Urban Development (HUD). <br /> <br />Purpose and Need: <br /> <br />· These funds will provide for the expansion of a special case management unit to <br />provide for the essential coordination, linkage, and monitoring of supportive services. <br /> <br />· The population to be served is disabled Portsmouth citizens. <br /> <br />Financial Impact: <br /> <br />· The Department of Behavioral Healthcare Services will provide a 25 percent cash <br />match.
