8.5.3. Records of service calls for repair and maintenance for a period of <br />three (3) years after resolution by Franchisee, indicating the date and time service was required, <br />the date of acknowledgment and date and time service was scheduled (if it was scheduled), and <br />the date and time service was provided, and (if different) the date and time the problem was <br />resolved; <br />8.5.4. Records of installation/reconnection and requests for service <br />extension for a period of three years after the request was fulfilled by Franchisee, indicating the <br />date of request, date of acknowledgment, and the date and time service was extended; and <br />8.5.5. A map showing the area of coverage for the provisioning of Cable <br />Services. <br />9. INSURANCE AND INDEMNIFICATION <br />9.1. Insurance: <br />9.1.1. Franchisee shall maintain in full force and effect, at its own cost <br />and expense, during the Franchise term, the following insurance coverage: <br />9.1.1.1. Commercial General Liability Insurance in the amount <br />of three million dollars ($3,000,000.00) per occurrence for property damage and bodily injury <br />and three million ($3,000,000) general aggregate. Such insurance shall cover the construction, <br />operation and maintenance of the Cable System, and the conduct of Franchisee's Cable Service <br />business in the LFA. <br />9.1.1.2. Automobile Liability Insurance in the amount of three <br />million dollars ($3,000,000.00) combined single limit each accident for bodily injury and <br />property damage. <br />9.1.1.3. Workers' Compensation Insurance meeting all legal <br />requirements of the Commonwealth of Virginia. <br />9.1.1.4. Employers' Liability Insurance in the following <br />amounts: (A) Bodily Injury by Accident: one hundred thousand dollars ($100,000.00); (B) <br />Bodily Injury by Disease: one hundred thousand dollars ($100,000.00) employee limit; and (C) <br />Bodily Injury by Disease: five hundred thousand dollars ($500,000.00) policy limit. <br />9.1.2. The LFA shall be included as an additional insured, as its interest <br />may appear under this Agreement under each of the insurance policies required in this Article 9 <br />except Workers' Compensation and Employers' Liability Insurance. <br />9.1.3. Upon receipt of notice from its insurer(s), Franchisee shall use <br />commercially reasonable efforts to provide LFA with thirty (30) days' prior written notice of <br />cancellation of any required coverage. <br />
