This division applies to the placement of new wireless support structures within a public <br />way. <br />Sec. 32 -361. — Wireless facilities franchise agreement required. <br />No person shall place new wireless support structures within the public way except pursuant <br />to, and in accordance with the terms of, a wireless facilities franchise agreement. <br />Sec. 32 -362. - Permit required. <br />Any party to a wireless facilities franchise agreement who desires to place new wireless <br />support structures within the public way must first obtain a permit from the city engineer in <br />accordance with this division. <br />Sec, 32 -363. - Application for permit. <br />(a) The city engineer shall establish a standard application form for permits under this section. <br />(b) The application fee shall be $250.00 apiece for each individual permit request included with <br />a single application. No application shall be reviewed unless and until the application fee has <br />been paid. <br />Sec 32 -364 - Materials to be submitted with application <br />(a) When filing an application, an applicant shall submit the applicant's name and a valid <br />electronic mail address at which the applicant may be contacted <br />(b) When filing an application, for each individual permit requested in the application the <br />applicant shall provide: <br />(1) The address and latitude /longitude of the location where the applicant intends to place the <br />wireless support structure; <br />(2) A description of any other regulatory review required for the site and the status of that <br />review (e.g.. Federal Communications Commission, Federal Aviation Administration. <br />NEPA impact report l: <br />(3) A statement and supporting documentation confirming that the wireless facility to be <br />attached to the wireless support structure and operation thereof_ will not materially <br />interfere with other pre - existing communications facilities; <br />4) Plans clearly depicting the location, dimensions and specifications, including materials, <br />of the proposed wireless support structure* <br />A -12 <br />
