This division applies to the placement of new wireless support structures within a public way. Sec. 32 -361. — Wireless facilities franchise agreement required. No person shall place new wireless support structures within the public way except pursuant to, and in accordance with the terms of, a wireless facilities franchise agreement. Sec. 32 -362. - Permit required. Any party to a wireless facilities franchise agreement who desires to place new wireless support structures within the public way must first obtain a permit from the city engineer in accordance with this division. Sec, 32 -363. - Application for permit. (a) The city engineer shall establish a standard application form for permits under this section. (b) The application fee shall be $250.00 apiece for each individual permit request included with a single application. No application shall be reviewed unless and until the application fee has been paid. Sec 32 -364 - Materials to be submitted with application (a) When filing an application, an applicant shall submit the applicant's name and a valid electronic mail address at which the applicant may be contacted (b) When filing an application, for each individual permit requested in the application the applicant shall provide: (1) The address and latitude /longitude of the location where the applicant intends to place the wireless support structure; (2) A description of any other regulatory review required for the site and the status of that review (e.g.. Federal Communications Commission, Federal Aviation Administration. NEPA impact report l: (3) A statement and supporting documentation confirming that the wireless facility to be attached to the wireless support structure and operation thereof_ will not materially interfere with other pre - existing communications facilities; 4) Plans clearly depicting the location, dimensions and specifications, including materials, of the proposed wireless support structure* A -12
