<br />UP-09-12 <br /> <br />1703.1707.1711.1713.1715.1717. <br />and 1721 Columbia Street <br /> <br />Lots 1 through 9 Block K Columbia Street and that portion <br />of Columbia Street closed beginning at the western line of <br />Williamsburg Avenue and proceeding westward <br />approximately 257.5 feet. Property is further described as <br />beginning at a point where the western line of <br />Williamsburg Avenue intersects the northern line of <br />Columbia Street, then proceeding 150 feet along the <br />western line of Williamsburg A venue more or less, then <br />proceeding westward 300 feet more or lees to a point, then <br />proceeding northwardly 204.51 feet more or less along the <br />western line of lot 9 block K and the eastern terminus line <br />of Columbia Street to a point in the northern line of <br />Columbia Street, then proceeding eastwardly 256.62 feet <br />more or less to the point of beginning. <br /> <br />3. That the use permit is approved subject to the following conditions which shall be <br />observed by the applicant and any successor in interest: <br /> <br />(a) Prior to site plan approval, a subdivision plat must be recorded which <br />creates a single undivided parcel of land. <br /> <br />(b) Prior to approval and issuance of a Permanent Certificate of Occupancy, <br />the Applicant shall submit an operational plan for approval by the Director <br />of Planning. Such approval shall not be unreasonably withheld. <br /> <br />(c) Prior to approval and issuance of a Permanent Certificate of Occupancy, <br />the Applicant shall submit a general business plan for approval by the <br />Director of Planning which addresses service provision and property <br />management. Such approval shall not be unreasonably withheld. <br /> <br />(d) Prior to approval and issuance of a Permanent Certificate of Occupancy, <br />the Applicant shall submit a detailed description of supportive services <br />that will be provided by the Applicant and a list of paid on-site service <br />providers for approval by the Director of Planning. Such approval shall <br />not be unreasonably withheld. <br /> <br />(e) The documents submitted under conditions numbered (b) through (d) <br />above and all subsequent material amendments thereto as approved by the <br />Director of Planning shall be so reasonably maintained and implemented <br /> <br />as a condition of this use permit. <br />
