AN ORDINANCE TO PROVIDE GROUP LIFE INSURANCE FOR cITY EMPLOYEES AND SETTING TERMS AI{D CO~IDITiONS UNDER WHICH E~PLO~EES ~%Y BE INSURED · Be it ordained by the Couatcil of the City of Portsmouth, Virginia: Section i. From and after the effective date hereof~ all employees and officers of the City of Portsmouth, including constitutional officers and their employees and the judges of the Courz of Hustings and the Circuit Court of the City of Portsmouth, shall be eligible for group life insurance as provided herein° Employees of the School Board also shall be eligible for coverage hereunder, pro- vided 75~ of all such employees shall apply for this insurance within six mont~S of the efxectiv date of ~le policy. Section 2. The amount of insurance each individual may obtain, payment of benefits, waiting periods, eivGence of insurability and other terms and conditions of the plan shall be in accordance with the specifications promulgated by the City Manager in requesting bids f~r such group insurance, the proposal of The Life Insurance Company of Virginia in response thereto, that company's policy form 989, Section S. E~ch employee applying for such insurance shall authorize the deduction from his pay at regular periods of amounts equal to sixty cents per month for each $1,000.00 of insurance coverage. The balance of premiums due under the policy of insurance shall be paid by the City of Portsmouth and all premium refunds, dividends, e~c., in connection therewith shall be the proper~y of the City° Section 4. The City retains the right to alser or cancel the plan under the terms and conditions stated in the policy of insurance° Section 5. This ordinance being necessary for the preservation of the public health~ morals and safety~ an emergency is declared to exlsm and it shall take effect from and after its passage.
