AN ORDINANCE TO PROVIDE GROUP LIFE INSURANCE FOR cITY EMPLOYEES <br /> AND SETTING TERMS AI{D CO~IDITiONS UNDER WHICH E~PLO~EES ~%Y BE <br /> INSURED · <br /> Be it ordained by the Couatcil of the City of Portsmouth, Virginia: <br /> Section i. From and after the effective date hereof~ all employees and <br />officers of the City of Portsmouth, including constitutional officers and their <br />employees and the judges of the Courz of Hustings and the Circuit Court of the <br />City of Portsmouth, shall be eligible for group life insurance as provided herein° <br />Employees of the School Board also shall be eligible for coverage hereunder, pro- <br />vided 75~ of all such employees shall apply for this insurance within six mont~S <br />of the efxectiv date of ~le policy. <br /> Section 2. The amount of insurance each individual may obtain, payment of <br /> benefits, waiting periods, eivGence of insurability and other terms and conditions <br /> of the plan shall be in accordance with the specifications promulgated by the <br /> City Manager in requesting bids f~r such group insurance, the proposal of The Life <br /> Insurance Company of Virginia in response thereto, that company's policy form 989, <br /> <br /> Section S. E~ch employee applying for such insurance shall authorize the <br /> deduction from his pay at regular periods of amounts equal to sixty cents per <br /> month for each $1,000.00 of insurance coverage. The balance of premiums due <br /> under the policy of insurance shall be paid by the City of Portsmouth and all <br /> premium refunds, dividends, e~c., in connection therewith shall be the proper~y <br /> of the City° <br /> Section 4. The City retains the right to alser or cancel the plan under <br /> the terms and conditions stated in the policy of insurance° <br /> Section 5. This ordinance being necessary for the preservation of the <br /> public health~ morals and safety~ an emergency is declared to exlsm and it shall <br /> take effect from and after its passage. <br /> <br /> Adopted by the Council on.
