(I) The name of the organization and the purpose for which it was organized. (2) The principal address of the organization, the address of any offices in the City of Portsmouth and its designated agent for process within the ~nwealth. If no such agent is designated, the organization shall be deemed to have deisgnated the Secretary of the Cc~onweatth. If the organization does not maintain an office, the name and address of the person having custody of its financial records. (3) The name and addresses of any chapters, branches or affiliates in the City of Portsmouth. (4) The place 1~here and the date when the organization was legally established, the folm of the organization, and a reference to any deter- ruination of its tax-exempt status under the Internal Revenue Cede. (5) The names and addresses of the officers, directors, trustees and the principal salaried executive staff officer. (6) A copy of a balance sheet and inccme stats~ent, with the opinion of an independent public accountant for the organization's immediately preceding fiscal year, or a copy of a financial statement certified by an independent public accountant covermng, in a consolidated report, complete infoImation as to all the preceding year's fund raising activities of the charit able organization, shc~5_ng kLnd and amount of .funds raised, provided, no charitable organization which has registered with the Manager for the cuI-rent and next preceding ~hree years, or exempt for such years under Section 57-40.3, Code of Virginia, 1950, as amended, shall be required to file any financial ir~foImation. The report required by this subparagraph shall cc~ply with the accounting standsrds prescribed pursuant to Section 25-35. (7) A-statsm~t showing the cc~putation of the percentages provided for in Section 25-37 and Section 25-48. (8) A statement indicating whether the org~anization intends to solicit contributiou~ fr~a the public directly or have such done on its behalf by others. (9) A statement indicating whether the organ4zation is authorized by any o~daer govermnental agency within the Cc~monwealth to solicit contributions and ~Jnether it, or any officer, professional fund-raiser or professional solicitor thereof, is or has ever been enjoined by any court or otherwise prohibited from soliciting contributions in any jurisdiction.
