A RESOLUTION ADOPTING A PLAN FOR THE LONG-TER~ CARE OF <br />THE IMPAIRED ELDERLY FOR THE CITY OF PORTSMOUTH, VIRGINIA. <br /> <br /> ~HEREAS, Section 2.1-373.7 of the Code of Virginia <br />directs localities to establish a Long-Term Care Coordi- <br />nating Committee for the purpose of developing, monitoring, <br />implementing and evaluating a plan for long-term care <br />for the impaired elderly; and <br /> <br /> WHEREAS, by resolution adopted on September 28, 1982, <br />this Council established the Long-TermCare Coordinating <br />Committee and designated the Department of Social Services <br />as the lead agency thereof; and <br /> <br /> WHEREAS, the Long-Term Care Coordinating Committee <br />was charged with the development of a plan to provide a <br />broad range of services to the impaired elderly to be <br />approved by this Council and submitted to the Long-Term <br />Care Council for the Co~nu~_onwealth of Virginia; and <br /> <br /> ~qER~AS, a plan for a continuum of long-term care <br />services has been prepared to benefit Portsmouth's impaired <br />elderly and prevent inappropriate institutionalization <br />of these individuals; and <br /> <br /> WHEREAS, it is necessary that this Council take official <br />action to adopt the Long-Term Care Plan as acceptable to <br />the co--unity and ensure that community-based resources will <br />effectively enable /~upaired elderly persons to remain <br />comfortably in their own homes for as long as possible and <br />to submit s.~e to the Long-Term Care Council of the Commonwealth <br />of Virginia. <br /> <br /> NOW, THEREFORE, BE IT RESOLVED by the Coun~l of the <br />City of Portsmouth, that Portsmouth's Long-Term Care Plan, <br />having been reviewed and considered aDnr~Driate to meet the <br />needs of the City's elderly populatio~ l~ adopted by this <br />Council, and the City Manager is hereby authorized to for~ard <br />such plan to the Long-Term Care Council of the Commonv~ealth <br />of Virginia. <br /> <br /> BE IT FURTHER RESOLVED that the Long-Term Care Coordinating <br />Committee is further charged with the responsibility to monitor <br />the implementation and operation of-the Long-Term Care Pt~n for <br />the Ci{y of Portsmouth, Virginia. <br /> <br /> Adopted by the Council of the City of Portsmouth, ~rgzn~a,~' ' ~ <br />at a~meeting held June 28, 1983. <br /> <br />Teste: <br /> <br />City Clerk <br /> <br /> <br />
