EXHIBIT A <br />Section 13-74 and Appendix A of the Code of the City of Portsmouth, Virginia are <br />amended as shown below. Deletions are indicated by double strikethroughs and <br />additions are indicated by double underlining. Except as expressly shown below, <br />Chapter 13 and Appendix A of the Code of the City of Portsmouth, Virginia are not <br />otherwise amended. <br />1. Sec. 13-74. Ambulance services fees established. <br />Fees are established for the following ambulance services as set forth in appendix A to <br />this Code, including evaluation, assessment, treatment, and ambulance transport: <br />(1) Nonemergency incident response and ambulance transport with basic life <br />support services. <br />(2) Emergency incident response and ambulance transport with basic life support <br />services. <br />(3) Nonemergency incident response and ambulance transport with advanced life <br />support Level 1 services. <br />(4) Emergency incident response and ambulance transport with advanced life <br />support Level 1 services. <br />(5) Emergency incident response and ambulance transport with advanced life <br />support Level 2 services. <br />(6) Emergency incident response and ambulance transport with specialty care <br />transport services. <br />(7) The fee for ground transport mileage shall be as set forth in appendix A to <br />this Code per loaded mile in addition to the service charges set out in <br />subsections (1) through (6) of this section. <br />(8) Emergency incident response and patient assessment with a medical <br />assessment and/or treatment performed but no transport to a health care <br />facility. This fee shall only apply if the Informed Consent to Refuse EMS <br />Care form indicates (a) patient treated and refused transport or (b) patient <br />refused care, but an assessment completed. <br />(9) Emergency incident response with a medical assessment and/or treatment <br />performed where the patient is determined to be deceased at the scene by a <br />medical provider. <br />(10) Patient care reports. <br />(11) Use of IV Kit (Pharmacy Kit 1). <br />(12) Use of Medication Kit (Pharmacy Kit II). <br />(13) Use of Narcotic Kit (Pharmacy Kit III). <br />(14) Use of Rapid Sequence Induction Kit (Pharmacy Kit IV). <br />
