a period of three years after the request was fulfilled by Franchisee, indicating the date of request, <br />date of acknowledgment, and the date and time service was extended; and <br />8.5.5. A map showing the area of coverage for the provisioning of Cable Services <br />and estimated timetable to commence providing Cable Service, if applicable. <br />9. INSURANCE AND INDEMNIFICATION <br />9.1. Insurance: <br />9.1.1. Franchisee shall maintain in full force and effect, at its own cost and <br />expense, during the Franchise Tenn, the following insurance coverage: <br />9.1.1.1. Commercial General Liability insurance in the amount of one <br />million dollars ($1,000,000.00) combined single limit for property damage and bodily injury. Such <br />insurance shall cover the construction, operation and maintenance of the Cable System, and the <br />conduct of Franchisee's Cable Service business in the LFA. <br />9.1.1.2. Automobile Liability Insurance in the amount of one million dollars <br />($1,000,000.00) combined single limit for bodily injury and property damage. <br />9.1.1.3. Workers' Compensation Insurance meeting all legal requirements of <br />the Commonwealth of Virginia. <br />9.1.1.4. Employers' Liability Insurance in the following amounts: (A) <br />Bodily Injury by Accident: one hundred thousand dollars ($100,000.00); (B) Bodily Injury by <br />Disease: one hundred thousand dollars ($100,000.00) employee limit; and (C) Bodily Injury by <br />Disease: five hundred thousand dollars ($500,000.00) policy limit. <br />9.1.1.5. Umbrella Liability Insurance shall be maintained above the primary <br />Commercial General Liability, Automobile Liability, and Employers' Liability policies required <br />herein. The limit of such Umbrella Liability Insurance shall not be less than two million dollars <br />($2,000,000.00) each occurrence and in the annual aggregate. <br />9.1.2. The limits required above may be satisfied with a combination of primary <br />and excess coverage. <br />9.1.3. The LFA shall be included as an additional insured under each of the <br />insurance policies required in this Article 10 except Workers' Compensation and Employers' <br />Liability Insurance. <br />9.1.4. Franchisee shall not cancel any required insurance policy without obtaining <br />alternative insurance in conformance with this Agreement. <br />9.1.5.
