UP -22 -20 <br />1820 Prentis Avenue <br />(Tax Parcel 0069 -0220) <br />ALL THAT certain lot, piece or parcel of land, with the buildings <br />and improvements thereon, currently known as 1820 Prentis <br />Avenue, situate, lying and being in the City of Portsmouth, State of <br />Virginia, designated as Disposition Parcel 3, Prentis Park <br />Conservation Project, as shown on that certain plat entitled "Plat <br />Showing Survey of Disposition Parcel 3, Prentis Park <br />Conservation Project, Portsmouth, Virginia, for the Portsmouth <br />Redevelopment and Housing Authority," made by AML <br />Surveying, Mortgage and Land Surveys, Portsmouth, Virginia, <br />dated August 27, 1993 and recorded in the Clerk's Office of the <br />Circuit Court of the City of Portsmouth, Virginia, in Deed Book <br />1114, at page 1712. <br />IT BEING the same property conveyed to The L. Louise Lucas <br />Trust by Deed from Southside Direct Care Provider, L.L.C. dated <br />December 24, 2009, and recorded in the Clerk's Office of the <br />Circuit Court for the City of Portsmouth, Virginia as Instrument <br />Number 100000009. <br />6. That the use permit is approved subject to the following conditions which <br />shall be observed by the applicant and any successor in interest: <br />(a) Without limitation, the use shall maintain compliance with the following <br />use - specific standards set forth in Section 40.2- 217(D)(2)(a) of the Zoning <br />Ordinance: <br />"(i) In addition to general requirements, halfway houses shall <br />comply with the standards set forth by the Director of the <br />Department of Corrections and the State Board of Corrections ". <br />(b) No more than eight (8) halfway house residents shall reside at the property <br />at one time and a minimum of one (1) "House Manager" shall reside at the <br />property at all times. <br />(c) Prior to Zoning Permit issuance, a Minor Subdivision Plat must be <br />approved by the City and recorded to vacate the lot lines between 1810 <br />Prentis Avenue (Tax Parcel 0069 - 0230), 1816 Prentis Avenue (Tax Parcel <br />0069 - 0240), and 1820 Prentis Avenue (Tax Parcel 0069 -0220) and <br />combine them into a single parcel. The Plat must comply with the <br />applicable requirements of Chapter 33.1 (Subdivisions) of the City Code. <br />
