(b) When filing an application, for each individual permit requested in the application, the <br />applicant shall provide; <br />(1) The address and latitude /longitude of the city facility on which the wireless facility is <br />intended to be co- located; <br />(2) <br />FederAl Comm-n' Atoms Comp A' 41,0AfinA 4 <br />Copies of any approvals for the site granted by a federal agency. <br />including conditions imposed by that agency. <br />(3) A statement and supporting documentation confirming that the wireless facility and <br />operation thereof will not materially interfere with other pre- existing communications <br />facilities; <br />(4) Plans clearly depicting the dimensions and specifications of the wireless facility, <br />including the antennae, base station, and all assorted wireless equipment; <br />(5) Detailed elevation drawings showing the co- location of the wireless facility, including <br />the base station and all other associated equipment, on the city facility; <br />(6) A proposed construction schedule for the installation of the wireless facility; <br />(7) A statement affirming that, to the best of its knowledge, the wireless services provider <br />or wireless infrastructure provider is not in breach of its wireless facilities franchise <br />agreement; and <br />(8) Such additional materials as are listed on the application form established by the city <br />engineer and are reasonably required by the city engineer to the extent permitted by <br />law to determine the approvability of a permit in accordance with section 32 -345. <br />(c) Any application not including all of the information listed in this section may be deemed <br />incomplete by the city engineer. <br />Sec. 32 -345. - Review and determination. <br />(a) The applicant shall provide such additional information as the city engineer may reasonably <br />request to the extent permitted by law during review of the application. <br />(b) For small cell facilities, the application review process shall comply with Section 32 -314. <br />"LO The city engineer shall deny a permit if: <br />(1) The proposed wireless facilities do not comply with applicable law and the terms of the <br />wireless facilities franchise agreement; <br />(2) The proposed wireless facilities have a negative aesthetic impact on the surrounding <br />area, considering the size and design of the facilities and the historic, residential, or <br />commercial character of the surrounding area; <br />(3) The proposed wireless facilities pose a risk to the public safety, including but not <br />limited to public travel within the public way; <br />(4) The proposed wireless facilities are inconsistent with the city's existing or planned <br />public safety communications system; <br />(5) The proposed wireless facilities pose a risk to the stability or efficacy of the city facility; <br />(6) The existing or planned uses of the city facility are inconsistent with the co- location of <br />the proposed wireless facilities; <br />
