Sec. 32 -342, - Permit reauired. <br />Any party to a wireless facilities franchise agreement who desires to co- locate wireless <br />facilities on city facilities must first obtain a permit from the city engineer in accordance with this <br />division. <br />Sec, 32 -343, - Application for permit. <br />(a) The city engineer shall establish a standard application form for permits under this section. <br />(b) Applicants may submit not more than 35 individual permit requests with each application. <br />(c) Applicants shall deliver applications to the city engineer or city engineer's designee together <br />with an application fee. No application shall be reviewed unless and until the application fee <br />has been paid. <br />(d) The application fee shall be $100.00 apiece for each of the first five individual permit requests <br />included with a single application plus $50.00 apiece for each additional individual permit <br />request above five included with a single application. <br />Sec, 32 -344 - Materials to be submitted with application <br />(a) When filing an application, an applicant shall submit the applicant's name and a valid <br />electronic mail address at which the applicant may be contacted- <br />(b) When filing an application for each individual permit requested in the application the <br />applicant shall provide: <br />(1) The address and latitude /longitude of the city facility on which the wireless facility is <br />intended to be co- located: <br />(2) A description of any other regulatory re_ view required for the site and the status of that <br />review (e.g., Federal Communications Commission, Federal Aviation Administration. <br />NEPA impact report): <br />(3) A statement and supporting documentation confirming that the wireless facility and <br />operation thereof will not materially interfere with other pre - existing communications <br />facilities: <br />4) Plans clearly depicting the dimensions and specifications of the wireless facility <br />including the antennae, base station, and all assorted wireless equipment: <br />5) Detailed elevation drawings showing the co- location of the wireless facility including <br />the base station and all other associated equipment, on the city facility <br />M <br />
