Any party to a wireless facilities franchise agreement who desires to co- locate non -small cell <br />facilities on existing structures in the public way must first obtai_n_a permit from the city engineer <br />in accordance with this division. <br />Sec. 32 -328, - Application for permit. <br />(a) The city engineer shall establish a standard application form for permits under this section. <br />(b) The application fee shall be $ 100.00 apiece for each individual permit request included with <br />a single application. No application shall be reviewed unless and until the application fee has <br />been paid. <br />Sec 32 -329 - Materials to be submitted with application <br />(a) When filing an application, an applicant shall submit the applicant's name and a valid <br />electronic mail address at which the applicant may be contacted• <br />(b) When filing an application, for each individual permit requested in the application the <br />applicant shall provide: <br />11 The address and latitude /longitude of the existing structure on which the wireless facility <br />will be co- located- <br />(2) The name of the owner of the existing structure and an agreement or other evidence <br />showing the owner has granted permission to the applicant to co- locate on the existing <br />structure, which evidence may include the owner's signature on the application: <br />(3) A description of any other regulatory review required for the site and the status of that <br />review (e.g., Federal Communications Commission, Federal Aviation Administration. <br />NEPA impact re <br />(4) A statement and supporting documentation confirming that the wireless facility and <br />operation thereof will not materially interfere with other pre- existing_ communications <br />facilities: <br />51 Plans clearly depicting the dimensions and specifications of the wireless facility, <br />including the antennae, base station, and all assorted wireless equipment: <br />61 Detailed elevation drawings showing the co- location of the wireless facility, including <br />the base station and all other associated equipment on the existing structure; <br />(7) A statement affirming that, to the best of its knowledge, the wireless services provider or <br />wireless infrastructure provider is not in breach of its wireless facilities franchise <br />agreement: and <br />A -7 <br />
