<br />(1) The name of the disabled veteran and the name of the spouse also <br />occupying the principal residence, if any. <br /> <br />(2) The address of the property for which the exemption is requested. <br /> <br />(3) Whether the real property is jointly owned by a husband and wife. <br /> <br />(4) A statement that the real property is occupied as the veteran's and/or a <br />surviving spouse's principal place of residence. <br /> <br />(b) The veteran shall also provide documentation from the U.S. Department of <br />Veterans Affairs or its successor agency certifying that the veteran has a 100 percent <br />service-connected, permanent and total disability. <br /> <br />(c) In the event that a surviving spouse of a veteran is claiming the exemption, <br />the surviving spouse shall also provide documentation that the veteran's death occurred <br />on or after January I, 2011. <br /> <br />(d) A veteran shall be required to refile for exemption from real estate taxes <br />only if the veteran's principal place of residence changes. <br /> <br />2.
