AN ORDINANCE TO AMEND SECTION 3-33 AND 3-34 OF THE CODE OF THE CITY OF PORTSMOUTH, VIRGINIA, 1973, PERTAINING TO COMMERCIAL RECREATION CENTERS. BE IT ORDAINED by the Council of the City of Portsmouth, Virginia: I. That Sections 3-33 and 3-34 of the Code of the City of Portsmouth, Virginia, be amended to read as follows: Section 3-33. Application for permit; contents. Application for such permit shall be made to the City Manager upon a form supplied for that purpose. The appli- cation shall contain the following information: (a) Name and address of the applicant, age, date and place of birth, and where pertinent, the relationship of the applicant to the company or corporation owning or managing the center. If a corporation, this information for the cor- porate officer signing the application, plus name and address of the registered agent~ date of incorporation and state of incorporation, and names of officers and directors thereof. Name and address of the owner of the machines and owner of the premises if different from that of the above. (b) The location of the center and the business, if any other~ conducted at the location. (c) Maximum n~er of patrons center will accommodate. (d) Maximum number of machines center will accom~modate and submission of necessary information in order to determine if there is sufficient space for such machines to prevent over crowding of users or interference with pedestrian circulation on the rest of the premises. (e) A certification by the applicant that all requirements of this Article have been complied with. (f) A list of all employees who are expected to serve in a managerial capacity in the center. Prior convictions of the applicant and managerial employees as well as any pending criminal cases involving such persons.
