81 <br /> <br />A RESOLUTION ESTABLISHING THE LONG-TERM CARE COORDINATING <br />COMMITTEE AND DESIGNATING THE DEPARTFzENT OF SOCIAL SERVICES <br />AS THE LEAD AGENCY THEREOF. <br /> <br /> WHEREAS, Senate Bill No. 219 adopted by the 1982 Session <br />of the Virginia General Assembly enacted Section 2.1-373.7 <br />of the Code of Virginia directing localities to designate a <br />lead agency and to establish a Long-Term Care Coordinating <br />Committee for the purpose of developing, monitoring, im- <br />plementing and evaluating a plan for the long-term care of <br />the impaired elderly; and <br /> <br /> WHEREAS, this legislation was enacted to assist in- <br />dividuals to remain in their own homes while assuring the <br />provision of appropriate care and services of such individuals, <br />including socialization, health care, nutrition, daily living <br />services and supportive assistance; and <br /> <br /> WHEREAS, in response to this legislative mandate, the <br />City Administration has met with representatives of the <br />Portsmouth Department of Public Health, Portsmouth Department <br />of Social Services and the Portsmouth Community Services Board <br />to discuss long-term care services in Portsmouth; and <br /> <br /> WHEREAS, it is clearly evidenu that the City of Portsmouth <br />currently provides services in each of the categories to its <br />impaired elderly residents, and that the task of the lead <br />agency in the Long-Term Care Coordinating Committee in Portsmouth, <br />will be to insure effective utilization of these services.. <br /> <br /> NOW, THEREFORE, BE IT RESOLVED that the Council of the <br />City of Portsmouth hereby establishes a Long-Term Care Co- <br />ordinating Committee consisting of representatives of the <br />Portsmouth Department of Public Health, the Portsmouth Depart- <br />ment of Social Services, the Portsmouth Community Services <br />-Board, the Southeastern Virginia Area Wide Model Program for <br /> Seniors (SEVAMP), and the Nursinq Home Pre-Admissions Screening <br /> Team for this City and designate~ the Department of Social <br /> Services to serve as the lead agency thereof. <br /> <br /> BE IT FURTHER RESOLVED that the Long-Term Care Coordinating <br />Committee shall have as its function the following responsibilities: <br /> <br />Develop a long-term care plan for ~pproval by <br />the City of Portsmouth and submission to the <br />Long-Term Care Council. <br /> <br />Coordinate local services so thau the unique <br />long-term care needs of the City are addressed <br />and gaps in services are identified. <br /> <br /> <br />
