}UB , ITTED BY <br />APPLICANT <br /> <br />UP-91-17 <br /> <br />EXHIBIT <br /> <br />LA MIRAGE HOME FOR THE ELDERLY <br /> <br /> La Mirage will be constructed on a three acre site at the <br />intersection of King Street and Louisa Avenue in Portsmouth, <br />Virginia. <br /> <br /> The three story facility will contain seventy-one (71) one <br />bedroom units, thirty-two (32) two bedroom units, four (4) <br />efficiency units and a resident marrager unit. In addition, there <br />will be a three thousand two hundred thirty -ninety (3,239 sq.ft.) <br />square foot dining area, lounges/day rooms, medical stations and <br />offices, laundry and other amenities as shown on the site plan. <br /> <br /> The operation of the facility will be by professional <br />management qualified in the care of the elderly pursuant to Social <br />Service Department requirements. <br /> <br /> The operation will employ approximately twenty-four (24) <br />people including full time nursing staff which will be available to <br />all residents of the facility on a twenty-four (24) basis. The <br />nursing staff will consist of at least one LPN and ~wo nurses aids <br />for each eight hour shift. The food service staff will consist of <br />a food service director and two dietary aids for each shift and <br />food servers. All residents of the facility will have access to <br />the dining facilities and food service. Maid service will provided <br />to the residents. A residential manager will be located in the <br />building. There will also be an administrative staff. <br /> <br /> This residential care facility will be an establishment for <br />the provision of residential, social and personal care for the aged <br />and special categories of persons with some limits on ability for <br />self care, with medical care provided. There will also be a <br />congregate housing component of the facility. Services provided <br />will include food preparation, dining and house keeping and medical <br />services for those persons who a~ unable to maintain completely <br />independent lifestyle but for whom more intensive care is not <br />required. <br /> <br />14 <br /> <br /> <br />
