UP-92-13 <br /> <br />Beginning on the west right-of-way line of Chowan <br />Drive at a point a distance of 180.05' north from <br />its intersection with the northern right-of-way <br />line of Airline Boulevard; thence southwesterly <br />157.65' to a point; thence northerly 39.95' to a <br />point; thence southwesterly 110' to a point; <br />thence northerly 54.93' to a point; thence <br />westerly 52.46' to a point; thence northerly <br />151.29' to a point; thence easterly 294.26' to <br />the west right-of-way of Chowan Drive; thence <br />southerly along said right-of-way 109' to the <br />point of beginning. <br /> <br /> 3. That the use permit is approved subject to the <br />following conditions: <br /> <br />(a) <br /> <br />The facility shall meet all local building codes <br />as determined by the Building Official and the <br />applicant or operator shall obtain a certificate <br />of occupancy before starting operation. <br /> <br />(b) <br /> <br />(c) <br /> <br />The applicant or operator shall obtain and <br />maintain a Virginia State Adult Day Care License <br />and comply with all applicable local, state, and <br />federal regulations. <br /> <br />The facility shall employ and be managed by a <br />full time, on site director who is a registered <br />nurse. <br /> <br />(d) <br /> <br />The director of the facility shall permit annual <br />inspections by the City's Office of Building <br />Inspections, Department of Environmental <br />Services, Fire Prevention Bureau and Health <br />Department or Community Services Board. <br /> <br />(e) <br /> <br />The facility shall be limited to providing <br />service to ambulatory adults, who shall not <br />exceed fifteen (15) in number. <br /> <br />(f) <br /> <br />A separate room or area shall be available for <br />participants to sleep and rest.
