A RESOLUTION DESIGNATING THE DEPARTMENT OF SOCIAL SERVICES AS <br />THE LEAD AGENCY AND CERTAIN MEMBER AGENCIES TO COORDINATE LOCAL <br />LONG-TERM CARE SERVICES FOR THE ELDERLY. <br /> <br /> WHEREAS, Section 2.1-373.7 of the Code of Virginia directs <br />localities to designate a lead agency and member agencies to <br />accomplish the coordination of local long-term care services <br />for the elderly; and <br /> <br /> WHEREAS, this statute was enacted to assist individuals <br />remain in their own homes while assuring the provision of <br />appropriate care and services to such individuals, including <br />socialization, health care, nutrition, daily living services <br />and supportive assistance; and <br /> <br /> WHEREAS, it has been determined to be in the best interest <br />of the City of Portsmouth and its elderly population that the <br />Department of Social Services serve as the lead agency in <br />coordinating such local long-term care services and that the <br />Portsmouth Department of Public Health, the Portsmouth <br />Community Services Board, the Southeastern Virginia Area-Wide <br />Model Program for Seniors (SEVAMP), the Mayor's Senior <br />Citizens Committee, the Portsmouth Redevelopment and Housing <br />Authority (PRFLA), and the local Nursing Home Pre-Admissions <br />Screening Team serve as member agencies in such coordination <br />efforts; and <br /> <br /> WHEREAS, it is required by Section 2.1-373.7 that the lead <br />and member agencies establish a long-term care coordination <br />committee composed of, but not limited to, representatIves of <br />the lead and member agencies to guide the coordination and <br />administration of public long-term care services for the <br />elderly in the City. <br /> <br /> NOW, THEREFORE, BE IT RESOLVED that the Council of the <br />City of Portsmouth, Virginia, hereby designates the Portsmouth <br />Department of Social Services as the lead agency and the <br />Portsmouth Department of Public Health, the Portsmouth <br />Community Services Board, the Southeastern Virginia Area-Wide <br />Model Program (SEVAMP), the Mayor's Senior Citizens Committee, <br />the PRHA, and the local Nursing Home Pre-Admissions Screening <br />Team to coordinate local long-term care services for the <br />elderly, having the particular responsibility under Section <br />2.1-373.7 of the Code of Virginia of establishing a long-term <br />care coordination committee with duties as set out above and in <br />said statute. <br /> <br /> <br />
