UP-99-04 <br /> <br /> Beginning at a point 60'+ west of the northeast corner of <br />the intersection Barton Street and Peninsula Avenue; thence, south, <br />108'+ to a point; thence, west 105'+ to a point; thence, north 108'+ <br />to a point; thence, east I08'+ to the point of the beginning. <br /> <br />3. That this amended use permit is approved subject to the following conditions: <br /> <br /> (a) Occupancy of the facility at any time shall be limited to a maximum of <br />twelve children with no more than five children being under the age of 2 ½ years. <br /> <br /> (b) The applicant shall maintain residency within the facility. <br /> <br /> (c) The hours of operation shall be from 7:00 A.M. through 6:00 P.M,, <br />Monday through Saturday. <br /> <br /> (d) Outdoor recreational activities shall be limited to the hours between 10:00 <br />A.M. and 6:00 P.M. The number of children allowed outdoors at any given time shall not <br />exceed eight. <br /> <br /> (e) Drop off and pick uP of children shall be limited to the front driveway <br />pad. <br /> <br /> (f) Staff parking and long-term visitors shall be limited to the rear parking <br />facilities. <br /> <br /> (g) On-street parking shall be prohibited. <br /> <br /> (h) The facility shall comply with all applicable federal, state, and local codes, <br />ordinances and regulations and shall obtain and maintain in effect all necessary licenses <br />and permits. The applicant shall obtain a Certificate of Use and Occupancy from the <br />Building Official <br /> <br /> (i) The facility shall be inspected annually by the Department of Social <br />Services, the Fire Prevention Bureau, and the Public Health Department and shall correct <br />any noted deficiencies. <br /> <br /> (j) All taxes and fees on the subject property and operation shall be paid <br />when due. <br /> <br /> <br />
